Key Responsibilities:- Support the individual in developing and maintaining a sense of purpose and belonging in his community.
- Encourage and facilitate opportunities for social interaction and engagement in activities that promote personal growth and well-being.
- Collaborate with the individual's family and other support services to ensure a cohesive and effective support plan.
- Provide transportation and support for the individual to access community resources and activities.
- A full UK driving license with the ability to drive a manual vehicle.
- Experience in social care or a related field (desirable but not essential).
- A willingness to learn and develop in the role, with a commitment to delivering high-quality support services.
