Key Responsibilities- Provide individualized patient care, assessing and addressing the physical, emotional, and social needs of patients.
- Develop and implement individualized care plans, working collaboratively with the multidisciplinary team.
- Conduct thorough assessments, identifying and prioritizing patient needs, and implementing interventions to achieve optimal outcomes.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Registered Nurse on the appropriate part of the NMC Register.
- Within two years of registration, aim to complete the Mentorship preparation programme.
- Achievement of indicators in KSF outline (subset 1 year, full 5 years).
- Continual update of KSF evidence portfolio.
- Evidence of Continuing Professional Development and maintenance of a Personal, Professional Profile.
