Change Grow Live is seeking a dedicated Healthcare Outreach Specialist to join our team in providing high-quality clinical services to individuals experiencing homelessness. As a key member of our outreach team, you will be responsible for delivering healthcare interventions, guiding service users toward a brighter, healthier future.

Key Responsibilities:

  • Provide clinical services to individuals in need, including health assessments, medication management, and health education.
  • Collaborate with our multidisciplinary team to develop and implement individualized care plans.
  • Conduct outreach and engagement activities to connect with service users and promote our services.
  • Participate in quality improvement initiatives to enhance our services and outcomes.

Requirements:

  • Registered Nurse with a valid nursing license.
  • Experience working with vulnerable populations, including those experiencing homelessness.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
