We are seeking a skilled and experienced Practice Nurse to join our Crosland Moor Surgery team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and working closely with our GPs, clinical staff, and administrative team.
Main Responsibilities- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide routine nursing care to patients as required, adhering to clinical guidelines and best practices.
- Deliver vaccinations and immunizations, as well as provide travel medicine services.
- Request pathology services as necessary and process results accordingly.
- Lead chronic disease clinics, delivering patient care and referring patients to secondary or specialist care when required.
- Maintain accurate clinical records, ensuring compliance with relevant legislation.
- Ensure effective use of read codes and maintain chronic disease registers.
- Provide NHS health checks and implement vaccination programs for adults and children.
- Prioritize health issues and intervene appropriately, recognizing and assessing patients with mental health needs.
- Support patients in the use of prescribed medications and provide education on healthy lifestyle choices.
- Collaborate with the clinical team on safeguarding matters, adhering to local and national policies.
- Deliver opportunistic health promotion and maintain baby immunization and smear results logs.
- Manage emergency drug kits and ensure a safe and efficient working environment.
Crosland Moor Surgery is a friendly, two-partner practice serving over 4,000 patients in the Crosland Moor area. We offer a unique opportunity for an experienced and motivated Practice Nurse to join our team and contribute to the delivery of high-quality patient care.
Job Requirements- Primary care experience as a Practice Nurse, with the ability to carry out wound care, chronic disease reviews, and immunizations for adults and children.
- Able to carry out asthma reviews and cervical smears (desirable).
