We are seeking a dynamic and forward-thinking Children's Community Nurse to join our team at Guy's and St Thomas' NHS Foundation Trust. As a Band 6 nurse, you will be responsible for providing high-quality specialist nursing care to children and young people with ongoing conditions such as asthma, eczema, and constipation.

The Patch Children's Community Nursing (CCN) team is part of the Children and Young People's Health Partnership (CYPHP), a quality improvement programme aimed at enhancing the health and well-being of children in Lambeth and Southwark. Our team provides community-based care through clinics located across multiple sites in the area.

As a Children's Community Nurse, you will work collaboratively with the team to deliver a population-based approach to healthcare, utilising progressive technology to deliver services. You will be responsible for assessing, planning, and delivering specialist nursing care, as well as providing expert nursing advice.

The ideal candidate will have experience working as an autonomous practitioner within acute, community, or primary care settings. You will have excellent organisational, time management, and communication skills, as well as the ability to work collaboratively as part of a team.

Main Duties of the Job

  • Facilitating early discharge and reducing hospital length of stay
  • Preventing hospital admissions by delivering proactive care in the community
  • Providing clinical supervision to staff and students
  • Engaging in research and leading clinical audits within your specialist area
