We are seeking an experienced District Nurse/Case Manager to join our Placed Based Team 4: Bedworth. This is a fixed-term, 12-month position that offers a unique opportunity to work in a dynamic and forward-thinking environment.
The successful candidate will have a general knowledge of current best practice in community nursing and hold a District Nurse degree. Ideally, you would have health assessment skills or be enrolled in a health assessment program. As a member of our team, you will be responsible for providing holistic care to housebound patients in their own homes, aiming to avoid unnecessary hospital admissions.
We provide a structured induction and training period to ensure your development in skills and knowledge. Continuing professional development opportunities are encouraged to support staff progression. The service operates seven days a week, and flexible working is required.
To be considered, you should hold a valid UK driving license and be willing to travel across North Warwickshire and surrounding areas. A mileage allowance is available.
Main DutiesThe District Nurse is responsible for assessing and managing the health needs of an identified population and developing, implementing, and evaluating programs of care within target groups, including end-of-life care, complex health needs, and patients at risk of hospital admission.
Required Skills and Qualifications- Registered Nurse (1st Level)
- Degree-level District Nursing Specialist Practitioner Qualification (or working towards)
- Leadership Qualification
A competitive salary of £35,745 - £44,503 per annum is offered, depending on experience. In addition to your salary, you will benefit from a contributory pension scheme, generous annual leave entitlement, and access to training and development opportunities.
