About the Role

We are seeking a highly skilled and compassionate Care Worker Facilitator to join our team at Acuity Care Group Limited.

This is a unique opportunity to work with individuals who require support with their daily lives, helping them to achieve their goals and live independently.

As a key member of our team, you will be responsible for providing person-centred care, working closely with service users, their families, and other professionals to ensure the best possible outcomes.

Main Duties

  • Support service users with their daily routines, including personal care and medication management;
  • Assist with meal preparation and planning, ensuring that individuals' dietary needs are met;
  • Provide transportation for service users, using your own vehicle and holding a full UK driving license;
  • Maintain accurate records of service user activities, progress, and any issues that may arise;
  • Work collaboratively with other staff members to ensure seamless delivery of care.

Person Specification

  • Experience working with adults with learning disabilities, autism, and mental health conditions;
  • Strong communication and interpersonal skills, enabling effective collaboration with colleagues and service users;
  • Ability to remain calm and composed under pressure, responding to changing circumstances with flexibility and adaptability;
  • Physical stamina to meet the demands of the role, including manual handling and standing for prolonged periods;
  • Full UK driving license and access to a reliable vehicle.
