About the Role

We are seeking a compassionate and dedicated individual to join our nursing team as a Holistic Care Practitioner. This role involves providing high-quality care to individuals living with dementia, mental health issues, and complex needs.

In this challenging yet rewarding position, you will work closely with our multidisciplinary team to deliver person-centred care that promotes independence, dignity, and well-being.

Key Responsibilities:

  • Providing holistic care to individuals with complex needs
  • Working collaboratively with the multidisciplinary team to deliver person-centred care
  • Participating in ongoing training and professional development opportunities
  • Maintaining accurate records and contributing to quality improvement initiatives

Requirements:

  • Nursing degree or equivalent qualification
  • Current NMC registration
  • Minimum 2 years' experience in social care
  • Excellent communication and interpersonal skills
