We are seeking a skilled Carers Support Officer to join our team at 21 HSC Limited in North Ayrshire, Scotland. The successful candidate will play a vital role in identifying carers across primary and acute health settings, providing information, support, and advice on carers rights in relation to the Carers Act (Scotland) 2016.

The ideal candidate will have a strong understanding of carers' rights under Section 28 of the Carers Act and experience working with unpaid carers or similar voluntary work. They will also possess excellent IT skills, particularly in utilizing the MS Office suite (Teams, Sharepoint, Word, Excel, etc.), and be able to maintain accurate records and data.

As a Carers Support Officer, you will work closely with hospital staff, social work teams, and other healthcare professionals to ensure carers are informed, involved, and supported throughout discharge planning processes for their loved ones. You will also establish and maintain links with GPs, Community Link Workers, Pharmacies, and clinics/health centers throughout North Ayrshire, referring carers to the appropriate locality Gateway Carer Support Officer as needed.

This is a full-time position, with a salary of £24,222 per annum, plus a 5% employer pension contribution. You will be required to attend team meetings within Health Teams to promote the work of the Carers Support Officer and the wider Carers Gateway service.

Please note that all Gateway staff are required to undertake out-of-hours provision on a rotational basis.
