Job Summary
This is an exciting opportunity to join our innovative Community Nursing Team in Macclesfield, Cheshire. As a Senior Community Healthcare Practitioner, you will play a key role in delivering effective nursing care to patients in their homes and within the community.
Main Duties
- Contribute to the delivery of high-standard nursing care as part of a multi-disciplinary team
- Assess and develop care plans for complex and simple care needs
- Delegated tasks include direct nursing care and administrative duties
About the Role
You will work closely with colleagues in Therapies and Social Services to deliver integrated care. This is a full-time position, with predominantly 9am-5pm shifts, although flexibility may be required. Travel is essential, and you must demonstrate how you would meet this requirement on your application form.
Requirements
- A registered nurse with experience in community nursing
- Ability to work autonomously and as part of a team
- Strong communication and assessment skills
