Field Care Manager

VP Complex Care is seeking a highly motivated and experienced Field Care Manager to join our team. As a Field Care Manager, you will be responsible for managing a combination of different care packages and clients living in their own homes.
Key Responsibilities:- Rota management and scheduling
- Risk assessment and care planning to meet individual client needs
- Supervision and appraisal of staff to ensure high-quality care delivery
- Client visits, spot checks, and care reviews to ensure client satisfaction
- Development of new care packages from local authority and private case management companies
- Supporting compliance of our field staff to ensure adherence to company policies and procedures
- Showing motivation and commitment to the role and encouraging this within the teams you are responsible for managing
- Supporting complaint handling and problem-solving to maintain the quality of care delivered to our clients
- Monitoring support packages, including reviewing support worker documentation
- Guiding staff through their own professional development to enhance their skills and knowledge
- Communicating effectively with clients, their families, staff teams, and our colleagues in the multi-disciplinary team
- Participating in the on-call rota to ensure 24/7 support
- 1 year of experience in care management
- NVQ level 3 or higher in Health and Social Care
- Ability to work on own initiative and react positively to changing working practices and environment
- Self-motivation and personal drive, with a team player attitude
- Good communication skills, both with internal and external customers
- Customer-focused approach
- Ability to maintain appropriate personal presentation and appearance
VP Complex Care offers a competitive salary range of £24,000 - £26,000 p.a. and a supportive work environment that encourages professional development and growth.
