The successful candidate will assist in conducting statutory Care Act Assessments for Older People and clients with Physical Disabilities, aiming to meet performance targets and reduce waiting times for new referrals.
Main Responsibilities:
- Completing six-week reviews to ensure continuity of care
- Conducting Mental Capacity Assessments to determine individual needs
- Performing Continuing Healthcare assessments to identify eligible clients
Requirements:
- Enhanced DBS clearance for working with vulnerable adults
- Knowledge of working with Older Adults and/or People with Disabilities
- Full UK Driving License for efficient travel
