About the Role

As part of our multidisciplinary team, you will provide timely patient-centred assessments to those with acute needs within the Queen Elizabeth Hospital. Your role will involve working closely with other healthcare professionals to reduce avoidable admissions by proactively assessing patients and addressing issues relating to function, mobility, and social needs.

Key Responsibilities
  • Provide effective and efficient multidisciplinary comprehensive geriatric assessment (CGA) within the Emergency department and the Acute Medical Unit.
  • Work autonomously and as part of a multidisciplinary team to provide high-quality patient care.
  • Supervise staff and support undergraduate students within their clinical placements.
  • Develop and implement care plans to meet the needs of older adults.
  • Collaborate with other healthcare professionals to improve patient outcomes and reduce hospital readmissions.
Requirements
  • Registered Adult Nurse on the NMC register.
  • 18 months band 5 experience.
  • Health care management/leadership qualification.
  • Health Assessment Course.
  • Specialist knowledge in the field of frailty syndromes & interventions for older people.
  • Experience of working at band 5 level or above within the NHS.
  • Experience in the supervision and management of junior staff.
