**Responsibilities and Accountabilities**
  • Assessing patients' needs and developing personalized care plans;
  • Implementing and evaluating care plans, making adjustments as necessary;
  • Collaborating with other healthcare professionals to ensure comprehensive support.
**Requirements and Qualifications**
  • A degree in Occupational Therapy or equivalent qualification;
  • Registration with the Health and Care Professions Council (HCPC);
  • Significant experience in a similar role within a community setting.
**Remuneration**

We offer a competitive salary of approximately £60,000 - £80,000 per annum, depending on experience, as well as opportunities for flexible working and professional development.
