This is an exciting opportunity to join our team as a District Nurse, working within the Out of Hours district nursing team based at Allan House Sowerby Bridge. As a key member of our community nursing team, you will be responsible for delivering high-quality, evidence-based nursing care to patients in their own homes.
Key Responsibilities- Deliver direct and indirect patient care, focusing on local needs and priorities, addressing the prevention agenda, and care centered on what matters to the person.
- Participate in the assessment, development, implementation, and evaluation of programs of care.
- Work collaboratively with patients, their families, and professional colleagues to provide a professional nursing culture that is open to change, new ideas, and innovation.
We are looking for a qualified and experienced nurse who is passionate about providing high-quality care and is committed to delivering complex care in the community. The ideal candidate will have:
- NMC Registered Nurse in a relevant field of practice (or within 6 months of completing a validated BSc or MSc course, resulting in NMC registration).
- Practice Assessor/Practice Supervisor qualification or willingness to complete.
