About the Role

We are seeking a highly skilled and experienced Occupational Therapist Specialist to join our Intermediate Care and Crisis Therapy Team at Chester le Street Community Hospital.

Key Responsibilities
  • Complete holistic and specialist assessments of patients with diverse or complex presentations, including those with physical disabilities and mental health issues.
  • Develop and implement rehabilitation plans to meet the needs of patients, including those requiring urgent community response therapy intervention.
  • Work collaboratively with multidisciplinary teams, including Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, and Assistants.
  • Lead or contribute to creating improvement, transformation, and innovation of services, including crisis intervention and community-based rehabilitation.
  • Manage crisis intervention within the community, meeting national guidance on two-hour responses, and provide holistic assessments, equipment, and short-term therapy to reduce hospital admissions.
  • Formally supervise lesser-experienced staff, assistants, and students, ensuring their input is within their role and competencies, and responsible for the safe use of equipment.
  • Participate in clinical governance procedures, including the prompt recording and reporting of incidents, equipment defects, and first-line management of informal complaints.
  • Appraise and develop clinical activities within the team, monitoring and reviewing individual and team performance, addressing performance capability issues where necessary.
  • Contribute to the provision of flexible and timely services, prioritizing workload to meet service demands.
  • Be responsible for own Continuous Professional Development, evidenced by attendance at relevant courses, achievement of appraisal objectives, and maintenance of a professional portfolio.
  • Participate in the staff appraisal scheme and Personal Development Plan as both appraiser and appraise.
  • Be an active member of the in-service training program, attending and participating in the provision of training to staff, attendance at training sessions, and practicing reflective practice.
  • Undertake the measurement and evaluation of work and current practices through the use of evidence-based practice projects, audit, research, and outcome measures.
Requirements
  • Comprehensive and relevant qualified experience in physical rehabilitation, including recent general rehabilitation experience.
  • Experience of collaborative working within multidisciplinary teams.
  • Evidence of communication skills within a wide variety of settings.
  • HCPC registered Occupational Therapist.
  • Ability to travel and carry small items of equipment on a daily basis to fulfill the requirements of the post.
  • Ability to meet physical demands of the post.
  • Commitment, motivation, and ability to work flexibly within a multidisciplinary service.
  • BSc Diploma in Occupational Therapy or equivalent.
  • Evidence of CPD, including attendance at recent post-graduate courses relevant to the clinical fields in the specialism.
