Job Overview

This is an exciting opportunity to join our Community Health Service team as a Community Nurse. You will provide holistic healthcare to patients in their own homes, residential care homes, and nurse-led clinics. We are looking for a highly motivated and enthusiastic registered nurse to complement our skill mix.

The successful candidate will have experience of working with professionals from other disciplines and be able to work effectively as part of a team. You will also have excellent verbal and written communication skills and be able to initiate patient assessment and care planning with the support of a senior colleague.
