Key Responsibilities- Provide patient care and education to individuals with peritoneal dialysis needs
- Participate as a member of the healthcare team and contribute to the development of individualized care plans
- Maintain competence and professionalism relevant to current practice
- Train patients, family members, and community agency staff on peritoneal dialysis and related care
- Perform peritoneal dialysis in the home environment and manage all aspects of care for the peritoneal dialysis patient
- Provide patient and family with advice, counselling, and teaching related to their condition
- Travel to complete home visits for patients and between Lakeridge Health sites as required
- Registration with the College of Nurses of Ontario (in good standing)
- Current B.C.L.S.
- Home peritoneal dialysis experience preferred
- Recent experience with the equipment related to home therapies, including peritoneal dialysis equipment
- Demonstrated aptitude for fostering client independence and training clients to manage self-care peritoneal dialysis at home
- Nephrology certificate or evidence of progression towards completion
- Demonstrated self-motivation regarding continuing education in areas of patient assessment, patient education, leadership, and related clinical specialties
- Sound knowledge and application of nursing skills as listed in the CNO 'Standards of Nursing Practice'
- Proven good organizational, interpersonal, problem-solving, and teaching skills
- Ability to prioritize and complete assignments in a reasonable length of time
- Demonstrated sound judgement, good observation, and patient assessment skills, tact, and empathy
- Ability to travel for home visits required
- Demonstrated ability to perform appropriately in emergency situations
- Physically capable of performing the essential duties of the position

- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area
