Job Description
We are an energetic service providing a comprehensive range of nursing, occupational therapy, and physiotherapy services, including specialist assessment, equipment, and short-term packages of care. Our innovative approach empowers patients to regain independence and autonomy, ensuring they receive the highest standard of care in their own homes.

The successful candidate will be responsible for working independently within professional guidelines, codes of practice, and organisational policies, demonstrating clinical effectiveness through evidence-based practice and outcome measures. Key duties include:
- Conducting comprehensive assessments, planning, implementation, evaluation, and auditing of clinical care provision
- Optimizing hospital bed utilization and reducing length of stay by ensuring timely, safe transfer/discharge of patients to agreed environments

Person Specification
The ideal candidate will hold a relevant degree with professional registration, possessing excellent leadership skills, experience in acute NHS/community settings, and proficiency in using modern technology. A bonus point for evidence of leadership courses/training.

Estimated Salary: £50,000 - £65,000 per annum.
