Reablement Officer

We are seeking a skilled Reablement Officer to join our team at Service Care Solutions Ltd.
Job Summary:
The successful candidate will work closely with our team to provide support and enable individuals to achieve maximum skills for independent living in the community.
Key Responsibilities:
- Support the team leader in ensuring that individuals receive person-centered care and support
- Provide direct management support to Support Workers
- Develop and implement goal plans and intervention plans that focus on outcomes
- Conduct specialist reablement assessments and provide recommendations for support
Requirements:
- Good understanding of current health and social care policies
- Ability to develop and implement goal plans and intervention plans
- Level 2 or 3 NVQ or other form of comparable training
Working Hours:
The successful candidate will work 2 shifts on a rolling rota, including weekends and bank holidays.
Benefits:
This role offers a competitive rate of £24 umbrella an hour (£19 PAYE and hour) and the opportunity to work with a dynamic team.
