We are seeking an experienced Team Leader to join our team and contribute to the development and coordination of supported housing schemes and the domiciliary service across Blackpool.

Key Responsibilities:
  • Oversee the deployment of staff to meet individual needs and preferences within available resources.
  • Supervise, coach, and support staff, volunteers, and students on placement.
  • Identify training needs of the team and ensure accurate records are kept.
  • Respond to referrals and undertake initial assessments of need.
  • Ensure all service users have a co-produced plan of support and a designated Key Worker.
  • Develop clear goals and expected outcomes for service users with a defined pathway towards greater independence.
  • Coordinate regular person-centred reviews that prioritize each person's views and unique journey.
  • Collaborate with local multi-disciplinary teams and community-based agencies to provide holistic, wrap-around support.
  • Maintain high standards of environmental safety and quality, ensuring a welcoming and inclusive atmosphere.
  • Facilitate service user engagement, involvement, and peer support.
  • Implement robust quality monitoring methods.
  • Provide key performance indicators and identify actions required.

You will provide excellent customer care and proactively communicate with stakeholders regarding risk management and the care and welfare of the people we support.
