About the Role

We are seeking an experienced Mental Health Practitioner to join our Integrated Co-occurring Needs Pathway (ICoN) team in South Staffs. As a key member of our team, you will work closely with clients experiencing complex mental health needs, providing evidence-based assessment and psychosocial interventions.

Key Responsibilities
  • Assess and formulate client needs, developing and implementing effective care plans.
  • Provide high-quality, evidence-based interventions, including psychological therapies and social support.
  • Liaise with local drug and alcohol treatment providers, enhancing client care and minimizing social exclusion.
  • Work collaboratively with the Integrated Co-occurring Needs Pathway team, promoting a holistic and strengths-based approach to client care.
  • Contribute to the development and implementation of innovative therapeutic models and interventions.

Person Specification

We are looking for a qualified Mental Health Practitioner with experience of working with adults experiencing complex mental health needs. You will have a strong understanding of evidence-based interventions and a commitment to delivering high-quality care.
