**Job Description:**
As a key member of our care team, you'll work closely with residents to understand their unique needs and preferences. Your duties will include:

• Providing individualized care and support to residents, including assistance with daily activities and personal care
• Collaborating with colleagues to develop and implement personalized care plans that meet each resident's goals and aspirations
• Encouraging residents to engage in social activities and pursue their interests
• Maintaining accurate records and reporting any concerns or changes in a resident's condition
