We are seeking a highly skilled and experienced Health Visitor to join our team in Slough Child and Family Wellbeing Service. As a Health Visitor, you will play a vital role in leading and delivering a high-quality, proactive service for children and families, working to reduce health inequalities and improve outcomes.
Key Responsibilities- Lead the health visiting team in Slough, working closely with other professionals to deliver the Healthy Child Programme and improve health outcomes for children and families.
- Develop and implement effective interventions to improve health outcomes for children and families, working in partnership with other agencies and stakeholders.
- Take responsibility for the outcomes of children on a child protection plan, working closely with key partners to address identified public health needs.
- Develop and foster effective professional relationships and networks with families and other key partners, providing a service that promotes the physical, mental, and emotional health and wellbeing of children and families.
- Co-manage a caseload within a team of other health visitors, acting autonomously and taking responsibility for the outcomes of those allocated for assessment, intervention, and management.
