About the Role

This exciting opportunity is for a highly skilled Telehealth Practitioner to join our team, working autonomously to assess, advise, and treat callers with a wide range of conditions. As a key member of our multidisciplinary team, you will utilize your strong clinical skills and sound judgement to provide safe and effective telephone clinical assessments.

Job Responsibilities

  • Undertake telephone triage assessments efficiently and effectively, identifying the most appropriate intervention to meet the needs of patients using best clinical practice and adhering to Trust Policy, protocols, and guidelines.
  • Work primarily in an autonomous capacity, assessing, advising, and treating callers within agreed guidelines.
  • Exercise professional autonomy and clinical judgement by critically analysing complex information utilizing clinical decision support software (CDSS), generic symptomatic assessment tools/frameworks, or the Emergency Communication Nurse System (ECNS).
  • Refer to other members of the multidisciplinary team as appropriate.
  • Manage calls safely, clinically, and situationally, based on caller's needs.
  • Identify emergency situations and redirect care appropriately.
  • Provide self-care advice to empower callers to manage their symptoms at home when necessary.
  • Support callers to CHOOSE WELL by advising on the correct use of primary care, emergency services, and other health or social care organizations.

What You'll Need

  • NMC Registered Nurse (adult or Child) or HCPC Registered Paramedic with demonstrable post-registration experience in front-line services.
  • Evidence of regular update training and continuous personal/professional development as required by the regulating body.
  • Able to demonstrate the Trust values and behaviours.
  • Strong listening and communication skills, empathy, and the ability to reassure patients and family members/friends in crisis situations.
  • Able to work independently, take instructions, and demonstrate team-working capabilities.
  • Sound clinical decision-making, risk management, and stress-defusing abilities.
  • High levels of current clinical knowledge, computer literacy, and a commitment to personal and professional development.
  • Flexibility to work rotating shifts, including nights and weekends, and travel to other sites as required.
