Job Overview

We are looking for a skilled and dedicated professional to support care leavers in their transition to adulthood. The successful candidate will work closely with local authorities and community organizations to provide high-quality services and ensure best outcomes for young people.

Key Responsibilities:
  1. Support Care Leavers: Deliver a high-quality service to care leavers, ensuring best outcomes and enabling them to succeed as they make their transition to adulthood.
  2. Plan and Implement Pathways: Regularly prepare, update, review, and implement pathway plans in conjunction with supervising managers, ensuring parallel planning where applicable.
  3. Human Rights Assessments: Undertake human rights assessments and ensure that the rights of young people are observed.
  4. Progress Monitoring: Keep informed about care leavers' progress and wellbeing, ensuring plans are progressed to meet assessed need.
Requirements:
  • Experience: Minimum 2 years working with young people in a supportive role in a voluntary or statutory setting.
  • Skills and Qualifications: Understanding of the Children (Leaving Care) Act 1989 and guidance relating to Care Leavers; knowledge of asylum, immigration, and human rights legislation; ability to empower and support a person-centred approach in the development, planning, and reviewing of support plans.
Salary and Benefits:
