Role Overview:

We are seeking a skilled and compassionate Clinical Care Specialist to join our team at Penrose Court Care Home. As a Clinical Care Specialist, you will be responsible for delivering high-quality, person-centered care to our residents, working closely with the care team to ensure seamless transitions and effective communication.

Key Responsibilities:

  • Care Planning: Collaborate with residents, families, and healthcare professionals to design and implement personalized care plans that meet the unique needs of each individual.
  • Leadership and Guidance: Lead by example, demonstrating clinical excellence and promoting a culture of respect and dignity within the care team.
  • Communication and Coordination: Ensure clear communication and continuity of care, facilitating smooth transitions and addressing concerns with empathy and urgency.
  • Health and Safety: Monitor and manage specific health needs, prioritizing resident safety and well-being.
  • Medication Management: Safely administer medications, adhering to MAR sheets and protocols.
  • Relationship Building: Foster strong relationships with residents, families, and healthcare professionals, addressing concerns and promoting a culture of respect and empathy.
  • Independence and Engagement: Support residents in daily activities, encouraging autonomy and promoting social interactions and activities.

Requirements:

  • A registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN.
  • A passion for delivering high-quality, person-centered care and a commitment to improving the lives of our residents.
  • Excellent communication and interpersonal skills, with the ability to connect with people from diverse backgrounds.
