Band 6 Community Sister: Hambleton and Richmondshire District Nursing

We are seeking a highly skilled and experienced Registered Nurse to fill a Community Sister position within our Hambleton and Richmondshire District Nursing team.

About the Role

This is an exciting opportunity to lead and manage a community nursing team, working closely with GP practices and other partners to deliver high-quality patient care. As a Community Sister, you will be responsible for providing exceptional clinical care, innovative practice, and support to the Clinical Lead and the team.

You will be expected to work flexibly to meet the changing needs of the service, which may include working across the Middlesbrough, Redcar and Cleveland, and Hambleton and Richmondshire area.

Main Responsibilities
  • Lead and manage a community nursing team, providing clinical guidance and support to team members.
  • Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals.
  • Collaborate with GP practices and other partners to deliver high-quality patient care, preventing unnecessary hospital admissions.
  • Contribute to the development of the team, identifying areas for improvement and implementing changes to enhance patient care.
  • Participate in quality improvement initiatives, championing best practices and promoting a culture of excellence within the team.
