Outreach Practitioner Job Description

This is an exciting opportunity for an experienced Outreach Practitioner to join Morgan Hunt UK Limited on a part-time basis. The successful candidate will be based in Wokingham and will be responsible for providing support to clients who are unable to access the service.

The estimated salary for this role is £24,000 - £30,000 per annum, dependent on experience.

Key Responsibilities:
  1. Community Outreach: Conduct home visits and provide support to service users in their homes.
  2. Client Re-Engagement: Visit clients who are off script/not in treatment to attempt re-engagement.
  3. Hospital Visits: Offer support to service users in hospital when necessary.
  4. Team Collaboration: Work collaboratively with partner agencies as required.
Requirements:
  1. Demonstrated ability to work with vulnerable populations, including those with complex needs.
  2. Proven experience in delivering high-quality services, including case management and record-keeping.
  3. Strong IT skills, including proficiency in Microsoft Office and digital meeting platforms.
