Palliative Care Team Manager

We are seeking a highly motivated and experienced nurse to join our Community Specialist Palliative Care Team as a Team Leader. The successful candidate will be responsible for managing the team, providing holistic needs assessment for patients, and offering advice and support to healthcare professionals.
Main Responsibilities- Lead the Community Specialist Palliative Care Team in providing high-quality patient care.
- Develop and implement care plans that meet the needs of patients with complex symptoms associated with advanced progressive disease.
- Collaborate with multi-professional colleagues to provide comprehensive and patient-focused care.
- Ensure that the team practices within a legal and ethical framework that adheres to The Code and local Trust Policies and Procedures.
- Significant experience of leadership and management in a community nursing setting.
- Master's level study in a relevant area of specialist nursing.
- Practice teacher or willingness to undertake.
- Previous management course/qualification.
- Nurse Prescriber V150/300.
- Ability to communicate complex information effectively.
- Ability to exercise critical thinking skills.
- Ability to analyse data and present information to various audiences.
- Ability to implement programmes of work leading to service changes.
- Demonstrates up-to-date evidence-based clinical knowledge in relation to community nursing.
