We are seeking an experienced and motivated Registered Nurse to join our Rehab and Frailty Team in the community. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient-centered care to individuals with frailty and long-term chronic conditions.
Key Responsibilities- Deliver comprehensive assessments and care plans for patients with complex needs
- Manage the deteriorating patient with frailty, including end-of-life care and advanced care planning
- Provide wound care, including complex and simple wound management
- Conduct observations and ECGs, as well as catheterization and venepuncture
- Utilize advanced physical assessment skills to identify and address patient needs
- Order pressure prevention equipment and provide comprehensive falls assessments
- Take medical and social histories, and complete comprehensive geriatric assessments
- Attend multidisciplinary team meetings and broad rounds to ensure seamless care coordination
- Liaise with other community teams and acute trusts to ensure effective care delivery
- Line manage junior members of staff and provide health promotion and education
- Refer patients to therapists for rehabilitation and support
- 2 years of NHS community experience
- Registered Adult Nurse, NMC sub-part 1 RNA (Adult)
- Current mentorship qualification
- Ability to travel to patient locations in a timely and efficient manner
- Vehicle license holder
- Trained in venepuncture
- Nurse prescriber
- Experience in frailty or elderly care
