Job Summary

We are seeking a highly skilled and experienced District Nurse to join our team at Manchester University NHS Foundation Trust. As a key member of our community health team, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.

Main Responsibilities
  • Lead and coordinate nursing teams to deliver complex nursing care to adults in their own homes and in community settings.
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care and coordination.
  • Develop and implement clinical skills and knowledge to improve patient outcomes and service delivery.
  • Provide leadership and mentorship to junior members of staff, promoting a culture of excellence and continuous learning.
  • Collaborate with the Band 7 team leader to ensure quality care delivery and service development.

Person Specification

We are looking for a highly skilled and experienced District Nurse who possesses the following qualifications and skills:

  • Registered Nurse qualification.
  • Community Specialist Practitioner qualification.
  • Prescriber V150 or V300 qualification.
  • Experience of working in community nursing and leading teams.
  • Excellent communication and leadership skills.
