About the Role
We are seeking a highly skilled and compassionate Clinical Assessor to join our Memory Service team at Sheffield Health and Social Care NHS Foundation Trust. As a Clinical Assessor, you will play a vital role in providing high-quality care to older adults with memory difficulties.
Key Responsibilities
- Assess, plan, implement, and evaluate treatment and collaborative care planning for service users.
- Provide cognitive testing and assessment in the community or hub base.
- Deliver group and 1:1 education interventions to support service users and their families.
- Maintain and promote a personalized approach in all aspects of service delivery.
Requirements
- Registered Nurse Mental Health or Occupational Therapist with 2 years post-grad experience.
- Working knowledge and understanding of older adults with organic mental health.
