Senior Care Coordinator Role Overview

We are seeking an experienced Care Coordinator to join our dedicated team in St Helens, Merseyside. As a skilled Care Services Coordinator, you will have experience in managing a caseload of care packages and understand the varied nature and fast-pace of the role, along with all elements of associated care delivery.

Key Responsibilities:

  • Manage rota's for care teams and respond efficiently to any day-to-day changes
  • Act as the first point of contact for service users, their families and other healthcare professionals
  • Apply gained communication and relationship building skills to effectively coordinate care services

About the Role:

This is an office-based role, Monday to Friday, 9am – 5pm but at times, travel across the Merseyside region will be required. You must hold a Driving Licence and have access to a vehicle. You will also be committed to supporting the out of hours on call service, which is covered on a 1 weeknight per week and 1 weekend in 4 rotational basis across the team.
