About the Role:

We are seeking a dedicated and compassionate Support Worker Specialist to join our team in Plymouth. As a Support Worker Specialist, you will play a vital role in providing high-quality care and support to individuals with learning disabilities in their own home.

Your primary responsibility will be to deliver personalized care and support plans, ensuring that each individual receives the necessary assistance to maintain their independence and reach their full potential.

You will work closely with a multidisciplinary team, including healthcare professionals, social workers, and family members, to provide comprehensive support and address any challenges that may arise.

Key Responsibilities:

  • To participate in the day-to-day implementation of individual support plans and activities within a challenging environment.
  • To maintain effective communication and liaison with service users, family, and others while respecting appropriate confidentiality.
  • To support service users with meal preparation, laundry duties, cleaning, and social events.
  • To maintain accurate notes and records as required.
  • To work as part of a team to provide high-quality care.

Requirements:

To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.

A diploma in health and social care or willingness to work towards one is essential. Ideally, you will have experience working in a care setting, but this is not essential.

You will also need to demonstrate the ability to remain calm under pressure and work effectively as part of a team.
