We are seeking a skilled Social Care Practitioner to join our team in the D2A North & West and Inner-City East. As a key member of our multidisciplinary team, you will work closely with internal and external colleagues to promote a whole service approach.

The role involves working under the statutory requirements of the Care Act 2014 to undertake support conversations, strength-based Care Act assessments, Carers Assessments and reviews with citizens in need of support. You will also be required to undertake support planning with service users who have eligible care and support needs to access the support they need and to meet agreed outcomes that promote choice and control and help them remain independent.

As part of your role, you will be undertaking Mental Capacity Assessments and Best Interests within the statutory requirements of the Mental Capacity Act 2005 with the support of qualified Social Worker where required.

The team has a diverse make-up of 1 team manager, 2 senior practitioners, 6 FTE social workers & 6 FTE social care practitioners. You will have regular formal reflective supervision by either manager or senior practitioner.

You will ensure citizens have the right professional expertise for their presenting needs at the earliest opportunity: preventing or delaying the need for care and support through maximising independence, autonomy, community engagement, managing or enabling risk, and health promotion.

You will have a full induction and opportunities for training and continuous professional development activities. The teams are primarily based in Bristol, we also operate an agile working policy which allows for home working.
