We are seeking a skilled Care Coordinator to join our team at Completely Care.

**Job Summary:**

The successful candidate will work in the Reading area, supporting young people and adults in independent living settings.

**About the Role:

This role involves developing life skills in areas such as rent arrears, debts, budgeting, social isolation, food and utilities or other basic essentials, with the aim of developing independence and preventing the risk of homelessness.

You will have excellent communication skills and be able to connect with a range of people from diverse backgrounds, including those affected by homelessness, mental health, alcohol/substance misuse, or ex-offenders.

To succeed in this position, you should have an understanding of the housing and welfare benefits system, homelessness, and mental health. Experience working with vulnerable populations is highly desirable.

**Responsibilities:**

- Develop and implement personalized support plans for clients

- Provide guidance on life skills, including budgeting, debt management, and finding employment

- Foster positive relationships with clients, families, and other stakeholders

- Collaborate with internal teams to ensure comprehensive support services

- Stay up-to-date with local authority policies and procedures relevant to the role

**What We Offer:**
