Job Title: Advanced Practice Nurse

An exciting opportunity has arisen for an experienced and skilled Nurse Practitioner to join our dynamic team in primary care. As a key member of the healthcare team, you will be responsible for providing high-quality patient care, working collaboratively with colleagues to achieve excellent health outcomes.

Salary: £45,000 - £60,000 per annum (dependent on experience)

Key Responsibilities:
  • Provide comprehensive assessment, diagnosis, treatment, and management of patients with acute and chronic illnesses
  • Develop and implement individualized care plans, taking into account patient preferences and values
  • Collaborate with multidisciplinary teams, including GPs, specialist nurses, and other healthcare professionals to ensure seamless care coordination
  • Maintain accurate and up-to-date records, ensuring compliance with relevant regulations and guidelines

Requirements: To be successful in this role, you will need:

  • Qualified as a Registered Nurse with current NMC registration
  • Relevant postgraduate qualification in primary care or advanced practice
  • At least 2 years' experience as a nurse practitioner in primary care
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse groups
  • Familiarity with relevant laws, policies, and guidelines governing healthcare delivery in the UK
