About the Role

We are seeking a highly skilled and dedicated professional to join our Assessment and Treatment Service in Bognor. As a Community Mental Health Nurse, Social Worker, or Occupational Therapist, you will be part of a dynamic multidisciplinary team providing specialist assessment, treatment, and care to individuals with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive clinical assessments, including risk assessments, to inform care planning and interventions.
  • Develop and implement individualized care plans, working collaboratively with service users, families, and other healthcare professionals.
  • Provide a range of clinical interventions, including therapy, counseling, and support, to promote recovery and independence.
  • Work effectively with other agencies and services to ensure seamless care coordination and transitions.
  • Participate in quality improvement initiatives and contribute to the development of service innovations.
Person Specification
  • Registered Nurse with current registration (mental health/LD/Adult) or equivalent qualification.
  • Significant post-qualifying experience in mental health care, with a focus on community-based services.
  • Experience of working with individuals with complex mental health needs, including those with severe and enduring mental illness.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work autonomously and as part of a team, with a commitment to continuous professional development.
