About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our Physical Health Team as a Clinical Nurse Specialist. The successful candidate will work closely with our multidisciplinary team to provide high-quality care to individuals with severe mental illness, focusing on improving their physical health and wellbeing.

Key Responsibilities
  • Provide clinical leadership and expertise to the Physical Health Team, ensuring the delivery of evidence-based care and interventions.
  • Contribute to the assessment of physical and mental health needs, with a focus on improving the physical health of individuals with severe mental illness.
  • Develop and implement care plans, working collaboratively with patients, families, and other healthcare professionals.
  • Support the smooth running of physical health clinics, including booking appointments, conducting assessments, and creating care plans.
  • Ensure compliance with NMC Code and Standards of Practice, as well as relevant legislation, policies, and procedures.
Requirements
  • Registered Nurse - Mental Health/Learning Disability/General, with current registration with the NMC.
  • ENB 998 / Preceptorship and or Practice Supervisor and Assessor Training.
  • At least 3 years' experience post-registration at Band 5, with community nursing experience.
  • Experience and skills in managing stressful situations, ensuring recognition and support to staff and service users.
  • Detailed awareness of relevant evidence base relating to physical health in SMI population.
