About the Role
Southern Health NHS Foundation Trust is seeking an experienced Advanced Nurse Prescriber to join The Willow Group, a forward-thinking and innovative GP group providing primary care services for 49,000 patients across five local sites in Gosport.
The successful candidate will be an enthusiastic, caring, and motivated professional with excellent clinical, organisational, and communication skills, able to work autonomously and as part of a team.
The post holder will support our on-going initiatives, including the Gosport Home Visiting Service and our Same Day Access Service, and will hold their own clinics, demonstrating critical thinking in the clinical decision-making process.
They will work collaboratively with the general practice team and extended primary care team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership.
Main Responsibilities
The post holder will be responsible for providing a responsive, safe, effective, and caring service to patients, requiring initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
They will demonstrate safe and effective clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The clinician will use a medical model to assess, diagnose, and prescribe for acute medical conditions, and will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

Person Specification
The successful candidate will have a relevant nursing/health degree, history taking and physical assessment course, clinical supervision training, independent and supplementary nurse prescribing course, and teaching and assessing qualification ENB 998 or equivalent/mentorship qualification PETALS.
