Palliative Care Specialist

We are seeking an experienced Palliative Care Specialist to join our team at Saint Francis Hospice. As a clinical leader, you will provide high-quality specialist support, advice, and symptom control to patients, their carers, and other healthcare professionals in the community.
About the RoleThis is a challenging and rewarding role that requires a senior nurse with extensive experience in palliative care. You will work collaboratively with the primary care team to deliver a 24/7 service, providing specialist telephone assessment and support, as well as face-to-face holistic assessments and monitoring. Your expertise will be invaluable in helping us to develop and improve our services.
Main Duties and Responsibilities- Provide specialist holistic telephone assessment and support ongoing for healthcare professionals, patients, and relatives.
- Work within community services to respond to referrals and patients on the telephone and visiting caseloads within allocated geographical areas.
- Liaise appropriately with primary health care teams to resolve problems identified.
- Carry out initial contact to all new referrals and re-referrals to community services.
We are looking for a highly motivated and experienced nurse who possesses excellent symptom control knowledge and current issues in palliative care. You should have evidence of recent and ongoing professional development in palliative care and be able to work autonomously as well as part of a team.
Required Skills and QualificationsThe ideal candidate will have:
- RGN with significant post-registration experience.
- Teaching Qualification or equivalent level of experience.
- Degree or equivalent level of experience.
- Significant senior post-registration experience at a similar level in palliative care or another associated specialty.
