We are seeking an experienced Adult Nurse Manager to join our progressive provider of complex care packages within the community. This role is based in the Surrey area, where we have secured further packages during an exciting growth phase.

About the Role

As a Senior Nurse Care Coordinator, you will be responsible for providing face-to-face support to adult clients and their families, creating personalized care plans for each person. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.

Key Responsibilities
  • Design and deliver tailored care plans specific to individual client needs
  • Effectively manage multidisciplinary teams through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
  • Deliver nursing interventions, including medicine management
  • Build and maintain relationships with clients' families
  • Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
Requirements
  • RGN with an active NMC PIN
  • Supportive management style that brings out the best in others
  • Up-to-date experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills, both verbal and written
  • Suitable for Band 5/6 Nurses with evidence of effective leadership and management of HCAs
