Job Overview

We are seeking an experienced Registered Mental Health Nurse to join our Community Enhanced Rehabilitation Team (CERT). The successful candidate will be part of a multidisciplinary team providing high-intensive community care for individuals with severe mental health problems.

The CERT team takes a formulation needs approach to address individuals' problems and difficulties, promoting a holistic view of recovery and personalized goals. As a keyworker, you will have a caseload and receive comprehensive supervision and training.

Key Responsibilities
  • Develop and implement integrated care pathways, including service user referrals and models of care.
  • Participate in case supervision to monitor standards of service user care and documentation.
  • Provide regular clinical supervision and ensure a safe clinical environment.
  • Manage medicines safely and adhere to the NMC code of professional conduct and Trust Medicines Code.
  • Assess care needs and develop, implement, and evaluate programs of care without supervision.
Person Specification
  • Up-to-date Professional Registration on the NMC register and current NMC registration.
  • Post-registration training in a relevant field.
  • Leadership/Management training.
  • Experience of mentorship, supervision, and coordination of staff.
  • Experience of working as a Nurse in Charge/Shift Coordinator.
  • Experience of Care Planning and the Recovery Model.
