About the Role:

We are seeking a qualified and experienced social worker to join our team at Tripod Partners. As an Adults Social Care Specialist, you will be responsible for conducting home visits to clients within the High Peak area.

Key Responsibilities:

  • Conduct home visits to clients within the High Peak area
  • Assess clients' needs and develop care plans
  • Work collaboratively with other professionals to ensure effective care delivery
  • Provide support and guidance to clients and their families

Requirements:

  • Qualified social worker registered with Social Work England
  • Experience of working with adults in a social care setting
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
