About the Role

We are seeking a skilled and dedicated Community Health Specialist to join our team at HCRG Care Group. As a Community Health Specialist, you will play a vital role in ensuring the smooth transition of patients from acute hospital settings to community-based care.

Key Responsibilities

  • Undertake patient care needs assessments to identify ongoing support required for safe acute hospital discharge.
  • Visit patients in their homes to undertake face-to-face assessments of patient needs.
  • Source appropriate care provision in the correct setting to meet these needs.
  • Ensure sourced care provision is implemented from the point of hospital discharge.
  • Communicate effectively with patients, carers, and all partner organisations.
  • Maintain accurate patient records in line with professional and organisational requirements.
  • Implement and work to ensure evidence-based care/best practice.
  • Initiate health promotion activities with individuals, patients, and carers.
  • Provide in-reach assistance to acute hospitals by providing expertise in community services to improve discharge and flow for pathways 1, 2, and 3.
  • Undertake mentorship training, facilitate the teaching and learning experiences for all students and colleagues.
  • Liaise with external agencies to expedite Transfer of Care Packages and Nursing Home/residential Home placements.
