Adults Learning Disabilities Team Social Worker Opportunity

We are seeking a skilled Social Worker to join our Adults Learning Disabilities Team in South Wales. This is a part-time position with the flexibility to work from home, offering a great work-life balance.

As a Social Worker in this team, you will be responsible for supporting individuals with complex needs, including those requiring Continuing Healthcare (CHC) and Community Care (COP) applications. You will work closely with our team to provide person-centered support and ensure the best possible outcomes for our service users.

Key Responsibilities:

  • Support individuals with complex needs, including those requiring CHC and COP applications
  • Work closely with the team to provide person-centered support and ensure the best possible outcomes for service users
  • Develop and implement support plans that meet the individual's needs and goals
  • Collaborate with other professionals, including healthcare and social care teams, to ensure a holistic approach to support

Requirements:

  • Experience as an Adult's Social Worker
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work part-time and from home
