Job Title: Primary Care Mental Health Practitioner

We are seeking an experienced Mental Health Practitioner to join our team as a Primary Care Mental Health Practitioner. This role will involve working within GP surgeries to provide mental health support to individuals with mild to moderate mental health problems.

Key Responsibilities:
  • Work with GPs and primary care teams to provide mental health support and interventions to individuals with mild to moderate mental health problems.
  • Conduct clinical assessments and develop care plans in collaboration with GPs and primary care teams.
  • Provide training and education to GPs and primary care teams on mental health topics.
  • Work closely with secondary care teams to ensure seamless transitions of care.
  • Maintain accurate records and reports of patient interactions and outcomes.
Requirements:
  • Registered Mental Health qualification or equivalent.
  • Post-qualifying experience of clinical work with people with a range of mental health problems.
  • Experience of partnership working within and across various different statutory and non-statutory teams and agencies.
  • Ability to work autonomously and as part of a team.
