As a key member of our primary care mental health team, you will play a vital role in delivering high-quality mental health services to patients in our community.
Main Responsibilities- Provide specialist mental health triage, assessment, advice, and/or signposting to patients in primary care.
- Work closely with local community teams, primary care multi-disciplinary teams, and primary care mental health hubs to develop creative solutions to meet minority group mental health needs.
- Champion the mental health needs of patients within our Primary Care Networks and wider service transformations.
- Develop and implement specialist mental health training within primary care surgeries.
- Professional registration in Occupational Therapy, Social Work, or Mental Health Nursing.
- Post-registration psychological training and clinical experience of brief interventions.
- Advanced understanding of mental health needs in adults and older adults.
- Experience of working in community mental health services for adults and/or older adults with severe mental health needs.
