About the Role
We are seeking an experienced and motivated Community Psychiatric Nurse/Care Co-ordinator to join our busy Community Mental Health Team for Later Life. The successful candidate will work with older adults living with dementia or functional illnesses, providing care coordination under the Care Programme Approach (CPA).
Main Responsibilities
- Provide care coordination under CPA, ensuring comprehensive assessments and care plans are in place for service users.
- Complete memory assessments, utilizing training provided to support service users and their families.
- Administer medication as prescribed, adhering to safe medication practices.
- Conduct initial assessments, gathering essential information to inform care planning.
- Participate in the duty rota, ensuring adequate coverage and support for the team.
- Conduct carer assessments, recognizing the vital role carers play in supporting service users.
